The East Sussex Cardiovascular Disease (CVD) Prevention Action Plan sets out a coordinated, system-wide approach to reducing the impact of cardiovascular disease across the county. Developed in partnership with local organisations, services, the plan focuses on prevention, early identification, reducing health inequalities, improving long-term condition management, and strengthening integrated care pathways. The action plan supports delivery of National and East Sussex priorities while recognising the financial, workforce and organisational challenges currently facing the health and care system.
